A man with a larger penis was considered to be a symbol of greater masculinity in history. The phallus is being worshipped as a symbol of creative energy in every world culture including India, Egypt, Greece, and Europe. Men are either proud or embarrassed of their penis size, shape and functioning. This often affects the self confidence of the person and it directly relates to female partner satisfaction, sexual performance, intimacy and love. Many surgeons thought for decades that Phalloplasty is useless and even impossible. But the achievements in the field of cosmetic and plastic surgery in the recent past proved that penis plastic surgery procedures are safe and effective. The number of people opting to get Phalloplasty surgery in India has increased tremendously to improve and rejuvenate their love lives. The psychological aspect of improving self-image is driving patients toward India rather than for a cosmetic change. Phalloplasty plastic surgery is an elective cosmetic surgery. The cost of Phalloplasty in India is 1/10th of the cost in other countries. The scope of Phalloplasty in India is increasing day by day due to its low cost and better healthcare facilities.
Penile reconstruction surgeries comprises of a wide variety of invasive procedures established as radical treatment for several congenital abnormalities of the urethra or penis. Penis modification could improve unsatisfactory sexual performances and relationship and intimacy problems. Phalloplasty refers to the construction or reconstruction of a penis or, often for cosmetic purposes. It is also occasionally called as penis enlargement. Penis plastic surgery enhances the length, girth, and/or the glands of the penis. The two holy grails of penis surgery include; pass urine from the tip and the second to be able to have penetrative sex. This plastic surgery is indicated in men when the penis is missing due to either congenital or acquired reasons, as well as in women with gender dysphoria. A complete construction or reconstruction plastic surgery of a penis is expanded to many disorders such as penile agenesis, micropenis, disorder of sexual development (intersex conditions), failed epispadias or hypospadias repair as well as penile cancer.

Neophalloplasty is one of the most difficult surgical procedures in genital reconstructive surgery. The important goal of the neophalloplasty is to construct the functional and cosmetically acceptable penis. Urethral reconstruction is the great challenge for surgeons who manage genital reconstruction in neophalloplasty. There are four different techniques for Phalloplasty. All of the techniques involve taking a graft of tissue from a donor site and extending the urethra. Different flaps such as penile skin, scrotal skin, abdominal skin, labial skin, vaginal flaps, etc. or grafts such as skin, bladder, and buccal mucosa have been suggested for urethral lengthening. Graft from the arm, graft from the side of the chest under the armpit, Graft from the leg or graft from around the pelvic bone can be used in Phalloplasty. An operation using the forearm as a donor site is the easiest to perform, but results in a cosmetically undesirable scar on the exposed area of the arm.

Stages of Phalloplasty:

Stage 1: It involves the formation of the phallus

Stage 2: This stage involves the formation of the urethra

Stage 3: Glans-sculpting, testicular prostheses etc are involved in 3rd stage.

Stage 4: It involves the penile prosthesis.

Phalloplasty needs an implanted erectile prosthesis to achieve an erection. This is usually done in a separate surgery to allow time for healing. Sometimes cleverly placed testicular prostheses hide the clitoris. Regardless of the type of Phalloplasty, implantation of prostheses is done once the urinary tract has been completed. A reservoir of the penile prosthesis together with testicular prostheses is implanted before the penile prosthesis itself is implanted.

Total vaginectomy is not necessary done either before or after Phalloplasty due to the associated heavy bleeding. Clitoris is preserved at the site for sexual sensation.

The lengthening of the urethra is when most complications occur. Surgery on cisgender men is simpler than on trans men, since the urethra requires less lengthening. The urethra of a trans man ends near the vaginal opening and has to be lengthened considerably. Scrotoplasty can be performed using the labia majora in all types of Phalloplasty in trans men, where prosthetic testicles can be inserted.

Post-operative depression requires temporary use of anti-depressants. Blood clot, shortage of blood supply, infection, death of parts of the penis or hematoma may occur as complications. There can be some scarring, though when done with lasers this is minimal. Loss of the phallus from either disease or blood supply issues can be another complication. Complications related to penile lengthening procedures include scrotalization where the penis is being covered by scrotal corrugated skin rather than by its natural smooth skin. Girth augmentation procedures may be complicated by loss of girth due to fat resorption, nodule formation due to uneven fat reabsorbtion.

The success rate of 80 to 90% is reported in terms of patient and partner acceptance, and almost 95% in terms of patient satisfaction.

Advantages of Phalloplasty Surgery:

  • The primary goal of Phalloplasty is to boost the self esteem of the person by modification of penis and also help in sexual enjoyment.
  • Phalloplasty can treat extreme penile curvature which makes sexual penetration painful.
  • New method involves smaller incisions and less scar tissue produced.
